Great news! We are now offering the My PMG Patient Portal to our established Pioneer Medical Group (PMG) patients. The portal is an online service that allows you to view your medical summary, communicate securely with your doctor, view lab and x-ray results and make appointment requests.

By using the My PMG Patient Portal, you can save time by doing common tasks from the comfort of your own home.

  • IT’S PRIVATE: Secure messaging ensures privacy between you and your physician
  • MORE CHOICE: Gives patients a new way to communicate with their doctor
  • SET & TRACK GOALS: Use the portal to track your health conditions/wellness goals and chart progress
  • PATIENT CONTROLLED: YOU own your portal and all of the data within it and control who sees what

Signing up is as simple as talking to a PMG receptionist when you visit one of our offices. Once we verify your information, you will receive an e-mail from noreply@followmyhealth.com inviting you to register. (Sign-up can only be done in person. Please remember to bring a valid I.D. with you.)

For your convenience, you can print the User Agreement form below and fill it out prior to your next appointment. You must bring this completed release along with proof of identity to your next doctor visit to register for the portal.

You can also register your children for the portal. If you have a child under the age of 12 and you would like to join the patient portal on their behalf you must fill out our Proxy Access for Children User Agreement and submit it to your doctor’s office during your next visit.
